If you wish to request a  Central Verification Organization Application with MetroCare & Health Choice or for one of our participating facilities, Methodist Hospitals of Memphis, MECH, Methodist Germantown Surgery Center, Methodist North Surgery Center, Methodist Fayette, LeBonheur East Surgery Center,  Hamilton Eye or Wolf River Ambulatory Surgery Center, please complete below and under comments state the Facility for which you wish to apply and your specialty.  If you do not want the application emailed to you, please provide your mailing address in the comment section as well.
